Our Diabetic Prevention Program is nationally accredited through the CDC and has been shown to help prevent and delay type 2 diabetes. The program is a year-long commitment that can be completed through our 100% virtual app or in-person.

What are the objectives of the Diabetes Prevention Program?

  1. To increase physical activity minutes to 150 minutes a week

  2. Prevent the onset of type 2 diabetes

  3. Help you create sustainable behavior changes 

  4. Regain the power of health

What can I expect?

The classes are 1 hour long and meet weekly to begin the program, later they move to monthly meetings as you advance in the program. Each class is led by a CDC Certified Diabetes Prevention Lifestyle Coach. You can choose to participate in the program through our partner app, HALT, or in person. 

When does the program start?

A new cohort starts on the first day of each month, you’re never too far away from a fresh start. Make sure to follow the instructions at the bottom of this email to get registered for the Diabetic Prevention Program. 

How do I know if I am qualified for the program?

If you haven’t already, you can find the risk assessment here, this assessment will give you a score based on your answers. Once you have taken the assessment, you will need to fill out this intake form. You will self-report on things like body mass index, A1C, and if you’ve experienced gestational diabetes. This information is confidential and will never be shared without permission.

What are the next steps after the Risk Assessment?

After you submit the intake form, we will contact you with Ground Zero Class times. 

Ground Zero Class is an informational class that will provide you with all the necessary information for your class. We will help you sign up on the app, get materials dispersed, and answer any questions you might have about the program. This is also an opportunity for you to meet your coach and the rest of your class. 

What is the cost of this program? 

Grace Fitness’ mission is to make health accessible to everyone, period. We have created a scholarship program, with the support of our community, to assist those in need. Please fill out this form if you are interested in a scholarship.

The cost of the year-long Diabetic Prevention Program is $300.00.
